At the end of a rehabilitative stay with us many patients discharge to home. If that is your plan, our social services staff will make the necessary arrangements for a smooth transition home.
For some patients, however, home may not be an option at this time, or they simply need a little more time to prepare for independent or semi-independent living. If your rehabilitation program is nearly complete but you are not quite ready for the next step, relax and stay a little longer as you work to become a little stronger. Whether it is for a few days, a few weeks or even longer there may be options for you to remain at the center. Our social service team is happy to speak with you about your options for remaining in the center, available payment options and taking part in the personal services available to you:
- Nutrition consultation
- Home assessment
- Nursing services
- Maintenance therapy assessment
- Recreational therapy and activities
- Spiritual services
- Scheduling doctor appointments
- Beauty/barber shop visits
- Home healthcare arrangements (if necessary)
