ProMedica Skilled Nursing and Rehabilitation (Libertyville) Receives Blue Ribbon in COVID-19 Vigilance Award

 

ProMedica Skilled Nursing and Rehabilitation (Libertyville) is honored to receive the Blue Ribbon in COVID-19 Vigilance Award from Telligen QI Connect™. This award recognizes ProMedica Skilled Nursing and Rehabilitation’s commitment to preventing the spread of COVID-19.

“We are honored to be recognized for our ongoing efforts in preventing the spread of COVID-19,” said Madiha Waheed, administrator at ProMedica Skilled Nursing and Rehabilitation, “Our dedicated employees are committed to delivering safe and quality care every day.”

Facilities on the recognition list have met criteria to prevent the spread of COVID-19, ensuring  the safety of their patients, residents and staff. This includes completing a COVID-19 preparedness attestation, achieving a staff COVID-19 vaccination rate of 95% or greater, a resident COVID-19 vaccination rate of 95% or greater, and a resident booster rate of 90% or greater.
